Host-side SqlAuthenticationProvider for Azure SQL Managed Identity authentication with Microsoft.Data.SqlClient 7.0+, using Azure.Identity's ManagedIdentityCredential directly instead of Microsoft.Data.SqlClient.Extensions.Azure (which pulls in the Windows-only WAM broker and crashes on Linux hosts).
